Our services not only focus on the treatment of existing conditions, but also on preventive medicine for the entire family.POSITION GOALTo provide safe and therapeutic management of patient care to an assigned group of patients through utilization of the Nursing Process, in adherence to the Standards of Practice and policies and procedures and in collaboration with other members of the health care team.EDUCATION and EXPERIENCE:Knowledge of the principles of growth and development over the life span and the ability to assess and interpret data, determine patient care requirements and deliver care relative to the patients-age specific needs of the adolescent, adult and geriatric patient population. Current California State Registered Nurse Licensure. BLS Healthcare Provider Status. ACLS Healthcare Provider Status. Documentation of completion of Basic Dysrhythmia course. Minimal of two to five years experience in clinical nursing practice in an acute hospital preferred Mechanical Ventilation competency assessed yearly. Specialty Infusion competency assessed yearly.PHYSICAL REQUIREMENTS AND WORKING CONDITIONS:Work is of medium demand, walking and standing most of the time on duty; fequent lifting of patients with assistance; frequent reaching and handling of instruments and equipment when attending to patient needs; works inside; varied temperature changes; contact with water/liquids; subject to noise and vibrations; hazards include exposure to blood and body fluids, sharp objects and instruments, assorted chemicals as listed in the MSDS manual; frequent contact with fumes, odors and exposure to vapors, mists and liquids; uses personal protective equipment. In this position, occupational exposure to communicable diseases and blood/bloodborne pathogens requires the use of personal protective equipment to include gloves, gowns, goggles, masks and radiation measuring devices, when in the clinical setting.BASIC RESPONSIBILITIES:Demonstrates knowledge of the principles of growth and development over the life span, assess as appropriate and interpret data, determine patient care requirements and deliver care relative to the patients’ age specific needs. When functioning in the Charge Nurse role, provides managerial support to the Clinical Director by supervising and evaluating all unit nursing personnel and monitoring delivery of patient care. Prioritizes patient care based on physical, psychosocial assessment and reassessment data. Plans patient care based on patient requirement/needs. Assures implementation of patient care based on identified needs and in a systematic and therapeutic manner. Assigns/delegates patient care according to patient requirements/needs and skill level of the support staff. Communicates significant patient status findings using appropriate channels and follows up on response. Implements appropriate emergency measures in crisis situations. Participates in telemetry observation and documentation. Demonstrates clinical skills at expected level of competency. Functions as a patient advocate for patient care concerns. Initiates a plan and participates in providing patient/family education according to the individualized needs. Functions within scope of practice and meets all legal and regulatory requirements. Assures that nursing care is documented systematically and legibly. Refers to and collaborates with other health care providers to plan patient care to achieve optimal patient outcomes. Identifies support staff skill and knowledge needs and participates in meeting these needs.As a Charge Nurse, makes decisions that are consistent with delegated authority and achieves expected results. Able to prioritize the needs of the unit. Takes accountability to control ordering and use of unit supplies. Assists in solving staffing problems that are within legal/ethical parameters and utilizes cost-effective resources. Assists Department Director with cost containment. Submits communication forms and records of conversation to keep the Department Director informed of unit and staff issues. Demonstrates good knowledge base of continuous quality improvement activities on the unit and house wide. Assumes responsibility for quality improvement activities assigned by Department Director. Demonstrates roll as change agent in influencing current nursing department issues. Assists Department Director in evaluating staff. Works with Department Director to implement disciplinary action consistent with organization procedures and guidelines. Helps staff to work to achieve unit goals. Assists Department Director in orientating new employees. Responds proactively to patient satisfaction concerns. Works with Department Director or House Supervisor to staff unit to meet patient care needs according to classification/acuity system, staff level of responsibility/ability and established patient care standards. Works with Department Director to initiate patient care conferences as needed. Works with staff to provide age specific, individualized patient care. Assists Department Director in arranging in-services, competencies and other education that will meet the needs of patient population served. Assesses nursing care needs of patients to assist in determining priorities for unit goals and performance improvement. Elicit patient/family cooperation in identifying needs and setting outcome. Demonstrates critical thinking in nursing interventions for the critically ill patient. Demonstrates clinical skills at expected level of competency. Ability to interrupt and intervene in hemodynamic calculations. Functions as a patient advocate for patient care concerns. Initiates a plan and participates in providing patient/family education according to the individualized needs. Functions within scope of practice and meets all legal and regulatory requirements. Assures that nursing care is documented systematically and legibly. Refers to and collaborates with other health care providers to plan patient care to achieve optimal patient outcomes. Provides documentation of continuing education, 15 hours yearly, with 7.5 hours relating to current area of practice. Directs emergency life support measures during a code situation. Coordinates the health team in all activities of an emergency situation. Participates in performance improvement/risk management activities. Demonstrates knowledge and application of principles of safety, environment of care, infection control, hazardous materials waste program and universal precautions, and performance improvement, information management, by completing the annual review process.
