Registered Nurse - Intake / Home Care

Assists the Administrator of Home Health evaluating systems to handle the management of patient referrals between the referral sources and the Home Health Care. Supervises reviews and evaluates referrals made by personnel on patients who may require post hospital health care. Remains current with changes in home health reimbursement and serves as a resource to hospital personnel, patient and family/caregivers.


Supervises Intake Coordinator staff. Participates as a representative of the Home Health Agency in designated activities such as meetings, health fairs, and promotional activities.


Essential Functions


Forms used for documenting referrals are evaluated and revised periodically.


Appropriate referral sources are contacted and evaluated. Assists the Administrator in creating a facilitating environment which encourages teamwork among staff. High levels of staff morale is created and maintained.


Follow ups on patients identified by referral personnel as potentially in need of home health care are done within twenty four hours of the referral.


Assures that assigned Intake Coordinators maintain productivity standards. Intake Coordinators maintain a productivity of 20-25 a month.


Maintains logs and records of Intake Assessment activity including cases assessed and not taken under care. Intake Records are maintained on a timely and accurate manner and demonstrate compliance with Home Health Services and Department of Health Standards.


Interviews the patient and family/caregiver in planning for facility discharge. When possible the patient is interviewed prior to facility discharge.


Make decisions to accept or deny admission for home health services according to Home Care policies and procedures; acceptance decisions comply with VCNY policies and procedures.


The patient's health insurance coverage is verified according to Home Care insurance verification procedure.


The patient's health insurance is considered in establishing the plan of care for home health services.


A positive working relationship is demonstrated in activities with professionals and community agencies.
* Interviews applicants and makes recommendations for hire.


Covers for the Intake Coordinators as necessary. The intake coordinators are to receive assistant/support from the Intake Supervisor.


Provides 24 hours professional telephone consultation and nursing. Calls received on the beeper are appropriately triaged.


Agency intake operations continue to run effectively in the temporary absence of Administrator Home Health Services.


Position is Monday - Friday... 9:00PM - 5:00PM

REQUIREMENTS
Requirements:


Must currently be a Licensed RN in the state of NY.


Min of 3+ years in Home Care (CHHA) with Supervisory Experience.
