Registered Dietitian - (Frederick) - (HE32743-3373-MD)
Registered Dietitian Exemption: Non-Exemptwork schedule is Mon-Sat(Varies).
Summary:
The Renal Dietitian provides services to the hemodialysis or peritoneal dialysis patients and their families in clinical and hospital settings to support and maximize the nutritional functioning and education of the patient.
Qualifications:
1) Bachelor's Degree in nutrition/dietetics or similar area.
2) Registered dietitian status with the American Dietetic Association or licensed in the practice of nutrition and dietetics if required in the state where the facility is located.
3) Minimum of one year experience in clinical dietetics and/or nutrition.
4) Good communication and analytical skills and ability to work with professional and non-professional personnel.
Job Functions:
Patient Care:
1) Assess nutritional status to determine appropriate nutrition intervention and recommendations.
2) Review medical and diet history, consult with medical staff, and the patient, documents appropriate information in medical record.
3) Develop industrialized Medical Nutrition Therapy including meal plan for each patient.
4) Provide nutrition education and counseling based on individualized patient needs including nutrient requirements, knowledge and abilities, medical, economic and social situation.
5) Select or develop education materials and methods appropriate for individual patient.
6) Monitor nutritional status, laboratory values, dialysis adequacy, adherence to diet and response to nutrition related medications and therapy.
7) Function as Bone Metabolism Specialist.
8) Review monthly lab results with each patient and/or caregiver.
9) Administer quality patient care in a professional manner.
Planning, Coordinating, and Organizing:
1) Revise and update nutritional recommendations and nutritional care plan as needed.
2) Evaluate outcomes and identify factors that may contribute to undesirable results and report appropriately.
3) Participate in rounds, monthly patient care conferences, continuous quality improvement (CQI) teams, facility team meetings and in-services.
Communication Skills:
1) Establish effective relationships with Medical Team, Nursing and other patient care teammates.
2) Establish effective relationships with patients and caregivers.
3) Contact and collaborate with extended care facility staff, family members or other appropriate nutritional care.
4) Effectively communicate pertinent nutrition issues with the medical team.
5) Learn and maintain basic computer skills as week as DaVita specific applications.
6) Present nutrition related in-service for teammates at least annually.
Documentation:
Document according to Federal, State and DaVita requirements.
1) Initial Assessment within 30 days of admission or transfer to facility or modality change.
2) Annual Nutrition Assessment at least yearly.
3) Nutrition Progress at least monthly.
4) Long-term care-plan at least yearly.
5) Short-term care plan monthly for unstable patients and every six months for stable.
Other duties as assigned:
1) May be required to work flexible schedule to meet patient care needs.
2) May be hired to work multiple facilities.
3) May be required to travel if required by coverage of multiple facilities or to attend meetings.
4) Required to attend initial training, Area Dietitian Meetings, and other DaVita specific meetings.
5) Understand and follow DaVita patient and employment policies and procedures.
6) Comply with all state or federal regulations of the Injury Prevention and Safety Training Program.
