Full Job Title: RN, Patient Care Coordinator - Riverside, Full Time -- Night ShiftClassification: NursingDuties and Responsibilities:
To function as a licensed professional under the supervision of the Dirctor of Nursing at Riverside. Utilizes leadership and clinical skills to coordinate patient care and unit operations. Delegates patient care responsibilities, facilitates communication among interdisciplinary team members and care teams, and manages the patient's plan of care to ensure achievement of desired patient outcomes and the delivery of cost effective quality care.Requirements: Bachelor's Degree in Nursing; 3 years experience as an RN in a Sub Acute or Acute care envionment required. An equivalent combination of education and experience may be substituted. Requires extreme flexibility with schedule to benefit needs of the patient population.
