Seeking a doc specialist, a manager to run a field staff from the main office, and a QI coordinator of HOME HEALTH
Clinical manager for a Major Home Care agency:
Manager candidate would ideally have Home Health, OASIS, Medicare exp, and case management..... BUT they would transition an EXPERIENCED (min 10 years) acute care charge RN to this role and orient. Join the leader in Home Health
Office based roles from the main office in North West Baltimore. You will assume leadership of a geographic area. manage practice standards, and aid in Case management., help in staff hiring and training. The'll manage OT, PT and RN's. This could be an OT or PT if they are GREAT and have exp in Home Health

POSITION TITLE: Quality Improvement/Staff Development Coordinator
To monitor, maintain, and improve the quality of services delivered to all customers by the VNA of Maryland in accordance with professional standards of practice, the corporate compliance and performance improvement programs. Assist in the development and implementation of components of Agency orientation and education programs. Act as a resource, mentor, and role model to all staff to enable professional growth and improved patient outcomes.
WORKING PROCEDURES AND/OR RESPONSIBILITIES:
1. Coordinate and report quality outcome information related to identified important aspects of care, patient occurrences, patient satisfaction surveys, infection control and other outcome data.
2. Monitor clinical performance improvement activities to ensure compliance with agency policies and procedures and standards established by accreditation and regulatory bodies and assist with the development of corrective action plans, where needed.
3. Report trends in documentation and patient care management to clinical management and subsequently develop educational programs to address deficits.
4. Assist with the development, implementation, and evaluation of the Performance Improvement and Corporate Compliance Plans.
5. Investigate and follow-up on patient grievances and risk issues in collaboration with clinical management.
6. Facilitate the utilization review functions within the Agency.
7. Develop, present, document, and evaluate educational programs to meet clinical staff needs related to but not exclusive to: Medicare guidelines, OASIS assessments, organizational changes, procedures, regulatory mandates, quality deficits or improvement, and documentation practices.
8. Seek expertise and input from clinical leadership in preparing staff development projects.
