Shft: PRN/On-CallJob Description:Appropriately delegates clinical procedures which do not require the skill/knowledge or judgment of a licensed professional and which may be safely delegated in accordance with Agency policies and procedures. Delegates activities to the Home Health Care LPN and/or Home Health Aide to carry out the plan of care. Supervises/monitors skilled care provided by the LPN as appropriate and as care dictates. Supervises the home health aide in the provision of unskilled services required by the patient. Develops, assesses, implements, and monitors a comprehensive plan of care for patients within an assigned case load and/or to patients, as assigned, for the team and for evenings, nights and weekends by coordinating an interdisciplinary team approach to service and resource delivery. The plan includes patient problems and goals as well as biophysical, psychosocial, environmental, long-term care, and discharge planning needs.
Completes a comprehensive, age-appropriate initial assessment based on the patients presenting problems, needs, and strengths and the physicians medical diagnosis. Contacts the physician when additional services such as therapies, social work, home health aide, or dietary may be required.
Conducts ongoing age-appropriate reassessments to determine response to care or services according to agency policy and/or changes in the patient or family status, re-prioritizes care based on new information, and updates the patients plan of care. Assures the patient and his or her family/caregivers participate in all assessment and planning activities.
Serves as an advocate and liaison between the patient/family and physician, Home Care team members, community resources, and third party payors as necessary.
Completes all physician orders (verbal, interim, certifications) within Home Care policy and requirements to reflect all services and disciplines provided. Performs therapeutic, age-appropriate nursing interventions as ordered by the physician and/or the nurse, consistent with scope of practice, standards of practice, agency policies and procedures, and individual competence, with consideration to risk management and infection control practices. Uses clinical knowledge and sound critical reasoning in care delivery and clinical decision making. Seeks guidance appropriately. Collaborates with all disciplines to collect, analyze, and monitor clinical outcome data for identified patient populations, which in turn will be used to improve patient care and identify structure or process mechanisms in need of revision. Coordinates, initiates and participates in interdisciplinary patient care conferences and shares patient, family, and physician concerns. Conducts ongoing patient/family education and communicates to other members of the health care team any educational needs, abilities, barriers and readiness to learn.Experience Required: At least three (3) five (5) years previous NICU, peds, med/surg, ER, ICU, or oncology experience. Preferred: Hospice, Home Care and/or intake/referral experience. Expertise in care planning, assessments, symptom and pain management and communications.Education/Licensure Required: Graduate of accredited school of nursing.
Licensure as registered nurse in State of Nebraska and Iowa.
Preferred: Bachelors degree in nursing.Knowledge/Skills/Abilities: Requires demonstrated abilities in effective utilization of all home health care services. Requires demonstrated skill in planning and coordination of client care.
Knowledge of managed care regulatory process including Medicare and Medicare. Excellent written and oral communication skills. Able to perform multiple responsibilities simultaneously.
Able to organize and prioritize. Knowledge of computers.
