The RN Director, Case Management will:
Integrates Utilization Management, Social Work, Continuing Care, Clinical Pathways, Quality Management, and Discharge Planning as is relates to Case Management activities.
Directs, plans, and supervises activities of case managers involved in following patients throughout the continuum of care to include strategic planning and daily management of operations related to case management for all inpatient, outpatient primary care, and chronic care units.
Completes needs assessments to determine service requirements of the target population. Develops and implements programs and makes recommendations concerning service delivery to meet these identified needs.
Oversees Case Managers in developing an individualized discharge plan and coordinating links between patients, family support systems, and community resources to enhance the patient’s optimum functioning in the community upon discharge.
Maintains liaison with physicians, hospital administrators, and ancillary services personnel to analyze unit needs, identify problems and evaluate overall unit performance. Effects changes as needed to improve services.

The RN Director, Case Management must possess the following:
Bachelor's degree required, Master's highly preferred.
Current licensure as a Registered Nurse in the State of Idaho.
Current BLS certification.
Seven years or more of clinical nursing experience, at least two of which must be in case management, care management, or utilization review.
At least three years of direct management experience in successfully managing a Care Management Department is required.
Prior experience in program development, monitoring and evaluating programs for compliance with established standards and mandates, and effective utilization of community resources.
Ability to demonstrate knowledge of medical terminology and community resources at City, County, State, and Federal levels. Solid, in-depth knowledge of InterQual Criteria, Medicare Part A and B.
