RN Coordinator - Home Care

Will evaluate and assess patient needs; coordination of services provided; patient and family education. Will maintain operations in compliance with DOH and ensure the delivery of quality patient care. Services provided to our Home Care patients. Required to conduct home visits.
REQUIREMENTS
RN licensure in New York State required; Minimum one year nursing experience; pediatric preferred. Experience in community health or long-term home health care.
