RN Case Manager/ RN Patient Care Supervisor

Community Hospices of America, a long-time premier provider of hospice care, is seeking qualified individuals to become a part of our team in the Shreveport/ Minden area to provide top quality, skilled nursing and supportive care to patients at the end of their lives. We are currently seeking:

RN Case Manager
Duties include performing assessments, care planning, and evaluating hospice home care for patients. Two years of
licensed nursing experience preferred.

RN Patient Care Supervisor
This RN will lead a multidisciplinary team of professionals while promoting a positive working relationship with patient families, physicians and other medical providers. Hospice, home health, or oncology experience required.
