RN Case Manager - Clinics

Responsibilities are to Provide case management and care coordination to geriatric patients at the clinic. Work with patients, clinicians to develop a plan of care to improve health outcomes and quality of life; perform duties specified by HealthEast's contract with UCare; includes utilization management and authorization of certain services. Qualified candidates must be a RN with geriatric experience; home care, nursing home and managed care experience; computer skills; ability to work autonomously; good assessment skills and knowledge of community services.We offer competitive compensation and benefits.
