Assists with the development, assessment, implementation, and monitoring of a comprehensive plan of care for identified populations by coordinating an interdisciplinary team approach to service and resource delivery. The assessment will include patient goals, as well as biophysical, psycho-social, environmental, and discharge planning needs. Maintains flexible work hours to accommodate patients and physicians which often include long hours and unscheduled shifts for emergency cases and peak periods of activity. Serves as an advocate and liaison between patient/family and physician, hospital staff, members of the health care team, community resources, and third party payers as necessary. Consistently evaluates the appropriateness of levels of care, diagnostic testing and clinical procedures, quality and clinical risk issues, and provides timely, accurate and complete medical record documentation and data collection which includes delay days.
Experience:
Three years of progressive clinical experiences as a registered nurse; Two years experience with disease specific population for a specialized care manager; Strong emphasis on critical thinking, diagnostic testing and surgical procedures.Education/Licensure:
Required: RN License in Nebraska or Iowa, Nebraska and Iowa are compact states so must be licensed in the state of residence. Bachelors degree required, BSN preferred.Knowledge/Skills/Abilities:
Job requires basic to moderate skills in reading, verbal and written communication. Strong leadership and critical reasoning skills in clinical decision making. Planing and organizing, initiative and ingenuity, creativity, independent decision making, problem solving, and ability to interpret data. Public relations and promotion of positive morale are important. Limited computer skills are necessary.
