As Care Coordinator, you will provide telephonic education and support to patients who are members of our health plan clients and customers. You will enjoy state-of-the-art computer technology and outcomes management systems (w/full initial 3-4 weeks training!) to enable the patient to accomplish improved health status goals.

REQUIREMENTS

Requires responsive, committed individuals to assist in the education, coordination of care and setting of behavioral change goals for patients, utilizing telephonic and computer technology to facilitate the positive interventions.
Qualifications include:
â?¢3 years current experience as an RN
â?¢Is able to manage medical co-morbidities.
â?¢Exemplary telephone etiquette and interpersonal oral communication skills a must.
â?¢Basic computer competencies in a Windows environment necessary.
