RN Care Coordination Supervisor ($1000 Sign-on Bonus)

The Geriatric Care Services Program provides care coordination and disease management services to frail older adults and their families, helping individuals navigate the complex health care system The GCS team of trained and specialized professionals help retired workers and their families obtain the appropriate, quality care they deserve. Our team plays a vital role in improving care, health and well-being of older adults.

Enjoy a weekday schedule with no weekends or holidays, while expanding your leadership skills as a RN Supervisor, Care Coordination. This experienced RN Supervisor will oversee a dedicated team of GCS nurse Care Coordinators providing leadership, technical direction and support. Duties include; organization of daily workflow, staff training, quality review, policy and procedure development and serving as a resource to the Care Coordination team. In addition, the RN Supervisor will assist with implementation for new business and client development activity.
REQUIREMENTS
•Valid and Unrestricted RN license in MN required.
•Bachelor’s degree in nursing required.
•Previous Supervisory and/or Management experience is required, preferably in an operations setting within health plan administration.
•Minimum 5 years clinical experience working with an older adult population, preferably in a community setting or long-term care facility. Additional experience with younger adult, disabled populations helpful.
•Minimum 2 years Care Management, Care Coordination or Disease Management experience ideal.
•CCM (Certified Case Manager) desired.
•Working knowledge of MS Windows.
•Strong customer service and organizational skills are essential.
