The Nurse Case Manager is responsible for managing the complex process of efficiently coordinating an individual's movement through the continuum of care. This is accomplished through effective intervention and communication with the individual, their family, physician, caregivers and payors in assessing individuals's needs, response to treatment, determining a target for length of stay based on medical diagnosis, and collaborating with other members of the health care team to develop and implement continued care plans based on the identified needs and goals for the individual.
