JOB SUMMARY:
Serves as the primary team leader, coordinating and monitoring the array of services and supports needed to address each person’s goals and desired outcomes as identified through the person’s self assessment, clinical assessment, and functional assessment. The QMRP assists each person in identifying and communicating his or her requests and needs for services and supports. The QMRP supervises, integrates and coordinates individual support plans, and monitors progress towards personal, clinical, and functional outcomes. The QMRP initiates periodic reviews, investigations, modifications and adjustments by soliciting the person’s feedback as the person wants and needs change.
JOB DUTIES:
1.Reviews assessments completed by the person and other members of the Interdisciplinary Team to support the person in identifying the supports and services needed to address their goals and desired outcomes. Completes Rights Assessment for each person prior to their Individual Support Plan (ISP).2.Supervises and writes the Individual Support Plan (ISP) based on assessments, interviews, and observations. Assesses progress by completing reviews, determining appropriateness of services, and by recommending changes as appropriate. Summarizes each person’s status and progress at least every 90 days.3.Oversees many aspects of group home activity including appearance of home, leisure materials, cleanliness and community integration.4.Spends time on a regular basis at the location where services are being performed. That means frequent oversight and work in residential homes, vocational centers, schools, and work sites including each shift, to ensure policy, procedures and program data are in place and being implemented appropriately.
The QMRP is expected to provide shift coverage in times of acute shortages of other direct support staff, but continues to be responsible for the primary management duties.5.Trains select components of Basic Clinical and Survival training and select aspects of Home Manager training. Trains and in-services direct care staff on new and changing programs to ensure correct and timely implementation.6.Works closely with other clinical/professional staff to maintain communication and provide feedback, standardize procedures, expedite support plan implementation and work-flow and to improve efficiency of subordinates.7.Conducts and/or oversees formal and informal investigations into incident/allegations of abuse, neglect, exploitation or other circumstances that may present risk to safety and health.8.Review incident reports to ensure treatment was provided, cause of injury determined, and preventative measures implemented. Analyzes incident reports for safety trends.9.Chairs interdisciplinary team meetings.10.Performs various assessments; chart reviews, medication administration observations, mealtime, environmental and zone monitoring.11.Schedules Quality Assurance assessments for each home monthly. Monitors for completion of assessment and addresses results with staff. Maintains a Quality Assurance notebook for each home.12.Oversees the admission and discharge procedures including case summaries and MEDICAID/MEDICARE processing.13.Works closely with the Administrator and Home Manager regarding employee work schedules, disciplinary actions, job performance and personal conduct. Attends Quality Management Team meetings.
14.Facilitates relationships between the company, parents and guardians. Obtains consents as needed.15.Participates on committees focusing on Human Rights, Quarterly Drug Review, Privacy Requirements, community and area mental health committees including school systems as appropriate. Periodically attends QI and Vocational Task Force meetings.16.Completes SNAPS, Medicaid Reviews and Utilization Reviews.17.Stays updated on RHA policies and procedures, service manuals and other regulations.
18.Drafts responses to and implements changes brought about by Quality Assurance audits and Department of Facility Services (survey) and/or other regulatory agencies. Seeks to maintain all services in compliance with all ICF/MR regulations without condition level deficiencies.

REQUIREMENTS
EDUCATION and/or EXPERIENCE:
Bachelor's degree (B.A./B. S) in Social Work, Social Services or Special Education from a four year college or university; at least two years experience working with persons with mental retardation or other developmental disabilities. Certain other Human Services Degrees may also qualify.CERTIFICATES, LICENSES, REGISTRATIONS:
Valid drivers license (this is a condition of employment) including personal vehicle insurance coverage. Must maintain proficiency in company sponsored training and certifications and privileging if required. The following are conditions of employment: CPR, first aid, NCI certification and others that may be specified and/or required by future company regulations. Must be qualified as a Habilitation Technician II (med certified) or eligible to be so designated at the first available training after accepting the position.
