POSITION SUMMARY:
An individual who is currently licensed in the state as a Physical Therapist in collaboration with multi-disciplinary team develops a plan of treatment for each physical therapy patient and provides skilled physical therapy services in accordance with physician orders.


JOB DUTIESProvides skilled physical therapy services/interventions in accordance with physician’s orders and plan of care.Assess patient needs initially and ongoing to determine level of functioning including OASIS assessment at appropriate times.Consults and coordinates with physician and interdisciplinary team to develop plan of care and evaluate team goals for each patient.Establishes therapy goals based on assessment and physician orders.Instructs patients/family/caregivers on patient’s total physical therapy program.Advises, consults and instructs members of the multi-disciplinary team in specific physical therapy modalities.Prepares and submits timely and accurate OASIS, clinical and progress notes, payroll and other required agency documentation.Coordinates services in consultation with other home health disciplines.Adheres to all agency policy and procedures while providing services to agency patients.Observes and reports patient progress and/or changes to physician and appropriate disciplinary team members.Participates in Multi-disciplinary team conferences and in-services as scheduled.Evaluates and provides supervision of Physical Therapy Assistants and Home Health Aides as appropriateEvaluates effectiveness and outcomes of care; modifies interventions to meet desired outcomes.Participates in discharge planning of patient from home healthcare.Performs other duties as assigned.Maintain complete confidentiality of personnel and financial information to which you are privilege.Actively contribute to the general well being of our working environment with a positive and constructive approach to our customers, fellow employees and tasks at hand.Assist in other projects as assigned.Acknowledges, understands and complies with the Compliance Program.
REQUIREMENTS
Essential Job Qualifications
Education: Graduate from a school of physical therapy accredited jointly by the American Physical Therapy Association and Council on Medical Education of the American Medical Associations.

Experience: Minimum of two years experience as a physical therapist in an acute care facility, outpatient setting, community health service or home health agency in the past three years.

Licensure/Certification: Current state license as a Physical Therapist. Current CPR. Valid drivers license. Current auto insurance. Current health certificate.

Other Qualifications: Ability to travel in service area.
