Physical Therapist - Full Time

Physical Therapist is responsible for assessment patients to determine needs for in house rehabilitation. Responsible for the assessment of clinical sytesm development of coordinated plan of care. Monitors and manages rehabilitative services of patients receiving in home therapy.
REQUIREMENTS
Graduate from an approved school of physical therapy and licensed practive in the State of Michigan. Previous home care experience preferred. Knowledge of home health/hospice regulations under and other third party reimbursement agencies. Ability to travel independently throughout the service area to patient's homes. Ability to access a patient and his/her equipment for purpose of assessment/treatment.
