will include evaluation and treatment of patients using age and diagnosis appropriate assessment tests and measurements. Candidate will be able to integrate assessment results to identify functional problems and prioritize individualized care needs with patient the family. Other job duties include being responsible for initiating therapy discharge planning in collaboration with care team and patient based on prognosis and expected discharged disposition.
