Personal Care Planner

Status: Open Department: Independent Living Power™

Purpose: Promotes independent living among SCAN® members by providing a comprehensive assessment of the member‘s needs leading to the development of a care plan.

Responsibilities:
Under the general supervision of the Manager, Independent Living Power™, the Personal Care Planner™ will be responsible for the following: work cooperatively and collaborate with other SCAN staff, with physicians, discharge planners, utilization management, home health agency and community agency staff and other professionals and paraprofessionals involved in the care of the member, and with the family to assure provision of high quality cost effective services. Perform initial assessments of members referred for Independent Living Power to determine eligibility for the Independent Living Power benefit and unmet needs, and conduct periodic reassessments of assigned clients, per departmental guidelines. Develop and implement care plans for assigned caseload, including needs and problem list and service plans, monitored and update as needed per departmental guidelines. In conjunction with Independent Living Power Assistant, prepare Purchase Service Authorizations, arrange, coordinate and monitor service provision for clients per department guidelines. Contact clients and their formal and informal support systems by phone, home visit and/or correspondence in order to monitor quality and quantity of service provision and monitor client’s status, according to departmental guidelines. Complete necessary documentation concisely, thoroughly and on a timely basis, per departmental guidelines, including computer entry of care plans, NHC certification and purchase service authorization.

Job Requirements



Requires BSW (MSW preferred) or related degree, geriatrics/medical healthcare experience and PC proficiency (including MS Office). Knowledge of Medicare criteria, medical terminology, case management principle/practices and superb assessment/interviewing skills desirable. Bilingual (Span/Eng) preferred. Involves local travel.
