Patient Care Coordinator - Medical Unit

Classification: NursingDuties and Responsibilities:
To function as a licensed professional caregiver under the supervision of the Manager in the 6C Medical Department. Utilizes leadership and clinical skills to coordinate patient care and unit operations. Delegates patient care responsibilities, facilitates communication among interdisciplinary team members and care teams, and manages the patient's plan of care to ensure achievement of desired patient outcomes and the delivery of cost effective quality. care.Requirements: Bachelor's Science in Nursing (BSN)Degree preferred; Licensed Registered Nurse with 2-3 years experience in the Medical Management of the acute stroke patient required. An equivalent combination of education and experience may be substituted. Requires extreme flexibility with schedule to benefit needs of the patient population.
