Lexington Health Care is proud to announce our new Post Acute Care Program. We are seeking individuals to lead, implement procedures, build strong relationships and partner with Physicians, and Physiatrists within this new setting.

Our goal is to rehabilitate our patients so they are able to return to their communities and familiar environments. As we have developed a post acute niche for our facilities in the Chicagoland area, the Post Acute Program Coordinator will demonstrate levels of expertise in the rehab, and cardio-pulmonary areas. These positions will participate on patient rounds with Physicians, and disciplinary teams, focusing on patient and family teaching.

Critical thinkers who possess strong clinical training skills and/or preceptor skills to train our staff to maintain a high level of competence will play key roles in our new program. The innovative, self-starting RN’s we seek will possess rehab certification and experience in an acute rehab setting. Bachelor’s preferred; acute care hospital, ortho certified, telemetry, med/surg, E.R. and/or sub-acute background desireable.
