Outreach Coordinator

Healthcare OmniCare, a Coventry Health Care Plan, is currently seeking qualified candidates for the following positions:

Supervisor, Complex Case Management Job Req ;16013) Responsible for the daily supervision of the Complex Case Management team; serves in an active managerial role to assist in the development, implementation, and evaluation of the disease management and case management process; reviewing the timeliness, appropriateness, and medical necessity of the case/disease management processes performed by staff. Oversees case/disease management activities relating to catastrophic cases and chronically ill members. Qualifications include a RN license for the State of Michigan, Bachelor's degree preferred; Minimum of three years of clinical and case/disease management experience. Previous supervisory experience required.

Complex Case Manager (Job Req ;15995) Responsible for comprehensive management of members with catastrophic illnesses, coordinating all case management activities including consultant referrals, home care visits and community resources. Qualifications include a current RN license for the State of Michigan and three or more years of clinical experience. Previous experience in case management and knowledge of community resources preferred. Bachelor's degree and CCM certification a plus.

Quality Improvement Coordinator (Job Req ;16017) Responsible for ensuring medical and health service compliance, performing a variety of administrative quality management duties for the Health Plan. Responsibilities include, but not limited to, facilitating and conducting implementation of medical record reviews (including HEDIS data collection and reporting) and facility inspections; investigating and monitoring quality of care issues; working with providers on development and monitoring of corrective action plans; working with the health service team colleagues on development and monitoring of process improvement initiatives; preparing outcome reports for committee review and presentation. Qualifications include a current Michigan RN license and a minimum of 2-4 years of clinical experience. Bachelor's degree preferred.

Outreach Coordinator (Job Req ; 13855) Responsible for coordinating and conducting telephonic member education, new member outreach, and targeted communications to members identified to have gaps in service interventions. Identifies opportunities to coordinate special events and presentations, such as health fairs, community services functions and orientations. Disseminates educational information to members and facilitates activations of any needed support and referral services. Serves as liaison with community service groups and resources, members, providers, and other customers. Qualified candidates must have a minimum of two or more years of experience in customer service, health education, or outreach. Must have strong communications skills. Medicaid managed care experience preferred.
