**On-site Case Manager RN (An autonomous position with growth!)

With this innovative role comes the opportunity to contribute directly to the success and development of a an exciting new program. For hospitals and skilled nursing facilities in the Dayton area, the On-site Case Manager works to coordinate the transitions between hospitals, facilities and patient homes, ensuring appropriate care is provided during all stages.



Primary responsibilities include:


Visiting hospitals to meet with the patient/member
Working with the physician, nurse and/or discharge planner and participating in the patient's transition to an acute rehab or skilled nursing facility
Monitoring the patient throughout the transition and conducting concurrent review, as well as on-site and telephonic case management
Coordinating authorizations for services
Providing updates and following up on patient status
Participating in team meetings at the facility
Building strong relationships with the facilities and staff


Job Requirements:

Requires RN licensure, a clinical background and case management experience in an acute rehab or long-term care setting. CCM preferred with working knowledge of Medicare regulations. Must be a self-starter who demonstrates strong interpersonal skills and excellent communication skills.
