Occupational Therapist - Home Care - Sussex County

Christiana Care VNA, the leader in Home Health Care, is seeking a Full Time Occupational Therapist to provide medically prescribed occupational therapy to patients in the community. The therapist makes the initial and periodic evaluation of the patient's physical and rehabilitation needs, participates in the development and implementation of the Plan of Treatment, in consultation with the physician and case manager as applicable, and treats the patient to develop or restore functions and maintain maximum performance.Qualified applicants will possess a current Delaware OT license and will also possess 2 years work experience as an Occupational Therapist. The qualified candidate will also have excellent communication/documentation skills and the ability to adapt to diverse home environments and family dynamics.
