ONCOLOGY CASE MANAGERS NEEDED IN ROCHESTER, NY!!!

Clinically assess, identify, coordinate, educate and case manage members with cancer and various co-morbidities while working cooperatively and effectively with staff to assure high quality, cost effective care.Bachelor's degree in Nursing

Minimum of 6 years experience, of which 3 years must be clinical working with individuals in ambulatory, acute or home care settings.

New York State licensure required
