Shft: DaysJob Description:Respects the privacy, dignity and individuality of each resident. Demonstrates timely response to resident calls, anticipates resident needs, and assures resident safety and comfort through regular rounds and care delivery. Performs individualized direct personal care including preparing residents for meals, feeding (also distributing nourishments and drinking water), toileting, grooming, bathing, and dressing. Assists in maintaining mobility and skin integrity by turning, positioning, range of motion, transfers, and locomotion. Assures resident privacy. Takes vital signs, measures intake and output. Contributes to and implements care plan under direction of RN / LPN team leader. Assists in restorative care by reinforcing self-feeding, dressing, bowel and bladder management, transfers locomotion, and the use of assistive device to promote optimal functioning. Reports to RN / LPN Team leader any changes in condition, patient incidents, the care given, observations, and resident requests. Demonstrates the ability to perform in an accurate and precise manner in crisis and emergency situations. Records appropriate information on assigned records.Experience Required: None. Six months for Rehabilitation Unit. Preferred: Previous experience in similar position.Knowledge / Skills / Abilities: Ability to organize and prioritize resident care and ability to work with others. Job requires continuous verbal communication skills. Job requires frequent reading skills and written communication skills. Must demonstrate personal traits of a high level of motivation, team orientation, professionalism, trust, and place a high value on treating others with dignity and respect.Educational Required:
Must be at least 16 years of age.
Must have successfully completed the 75-hour state approved course for Nursing Assistant.
Nurse Assistants must be on the Nurse Aide registry of the State in which they work.BLS Basic Life Support course completed within 3 months of hire.
