St. Mary Medical Center is a Regional Cancer Center affiliated with the Fox Chase Cancer Network and a nationally recognized Primary Stroke Center.

This position is Full Time, Monday-Friday, 8:00a-4:30p.You will work with a group of Internal Medicine doctors. In addition to focusing on effective communication with the physicians, patients, and patient families, you will:
•Perform comprehensive daily assessments focusing on real, potential, acute and/or chronic health problems.
•Maintain compliance with CMS Quality Indicators, JCAHO Core Measures, and other clinical indicators.
•Record and document health assessment data.
•Facilitate admission orders, treatments and appropriate referrals in a timely manner.
•Interpret and evaluate diagnostic tests to identify and assess patients’ clinical problems and comprehensive healthcare needs for further evaluation by the physician.
•Facilitate patient discharge orders, assist with discharge instruction sheets, and notify family physician of patient’s discharge.
