Nurse Practitioner (Very autonomous role/Set your own schedule!)

We're looking for a talented Nurse Practitioner to join us in this autonomous role. We offer orientation to long-term care, including preceptors and ongoing support. Make a direct impact in the lives of the people we serve and enjoy the freedom to set your own schedule when you apply your expertise with us.



Primary responsibilities include:
Making on-site visits to long-term care facilities to provide care to aging residents
Managing chronic conditions as necessary
Working actively and collaboratively with physicians to manage a caseload of 80-100 patients on a monthly basis
Building relationships and maintaining positive communication
Setting up appointments and managing a caseload
Making rounds with physicians
Participating in care conferences with patients and families


Job Requirements:

The successful candidate will be a licensed NP with national certification in family, adult, geriatric or acute care. Long-term care experience with geriatrics and an NP is highly preferred. New NP's with significant RN/long-term care experience will be considered. A DEA license would be helpful. Good computer skills are essential.
