Nurse Practitioner, Transplant Clinic, FT, Days

The Transplant Nurse Practitioner practices as an independent care provider in collaboration with the transplant physicians, including the Chief of Transplant, primarily on the inpatient Renal & Transplant Unit and as requested in the Transplant Short Stay Unit and out-patient settings in accordance with the Nurse Practitioner/Clinical Nurse Specialist Act of New Jersey (PL.1991, Chap 377). Her/his practice is characterized by comprehensive health assessments and management of patients with acute and/or chronic renal disease. The Nurse Practitioner shall have clinical responsibilities inclusive of transplant candidates, living donors, post transplant patients and patients enrolled in research trials. Obtains patient histories and performs physical examinations. Writes admitting orders, routine orders and daily progress notes as indicated. Acts as liaison between Saint Barnabas and all referring dialysis centers, community agencies and referring physicians. Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department. Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provides age specific treatment. Certified Nurse Practitioner licensure in the state of New Jersey. Registered Nurse in the state of New Jersey. Certified Clinical Transplant Coordinator (CCTC) certification preferred. Experience with ESRD population and /or transplantation preferred. Must seek and maintain current certification in the following: IV Therapy, Basic Life Support (BLS), Phlebotomy.
