Rapid expansion has created a need for us to hire a new Nurse Practitioner (NP) to work with geriatric patients in the field, around the Chicago area. One nice thing about this position is the small caseload, allowing you the opportunity to build real relationships with your patients. It is also a generally autonomous position, within the framework of a Fortune 100 healthcare organization. The position combines direct care with care coordination and program enhancements.

Key responsibilities include:


Seeing approximately 8 patients per day in two different facilities


Working with the patients' families, primary care physicians and facilities staff to coordinate care


Educating facilities' staff and enhancing the company's image
Transferring sales leads to the sales staff and/or Account Executive

Job Requirements:

Only nationally certified Nurse Practitioners with extensive experience will be considered for this position. An MSN degree is also required. Basic computer proficiency is essential. NPs with adult, family, geriatric or psychiatric certification are preferred, as are those with nursing home experience.
