We're looking for a Nurse Practitioner to provide primary care services in a collaborative environment for frail and elderly patients in a long-term care setting. Our Nurse Practitioner and a Primary Care Physician form a primary care team that works with the nursing home staff to help maintain the health and well being of our enrollees. This is an ideal position for someone who enjoys working independently.

Primary responsibilities include:
Communicating with physicians, nursing home staff, members and members' family
Providing primary care that will focus on assessment, health management, education, advocacy and prevention
Entering data and health history information into a web-based application


Job Requirements:

This position requires recent, professional experience working in a long-term care setting and a Masters degree in Nursing, specializing as a Nurse Practitioner. State license and national certification is required. Computer proficiency is also required. New graduate Nurse Practitioners with a background working in long-term care as a registered nurse are encouraged to apply.
