Nurse (MR/DD Waiver Program)

Description:
Reports to Director of MR/DD Waiver Program Nurse is responsible for reviewing service requests and making determinations for service arrays, including nursing related services/conditions. The Nurse communicates with the Service Coordinators/Providers and/or designated team regarding consumer needs, including recommendations for additional or alternative services. Plans are also reviewed to determine if they meet state guidelines and conform to person-centered planning. REQUIREMENTS
Requirements:
Experience in public health, community nursing, outpatient clinic settings, residential care or in-home care Minimum 2-3 years experience in assessment and service delivery for individuals with MR/DD Experience in problem resolution Strong verbal and written communication skills Customer service focus
