Our Field Case Manager's (FCM's) are dedicated healthcare professionals with a focus on providing quality care. You will provide on-site and telephone case management for clients in the Price George, Anne Arundel and Baltimore counties of MD with complex medical needs.

- Registered Nurse or Lic. Social Worker (LSW) in the state of Maryland

Must have case management and/or pediatric experience.

Need good organization, communication and computer skills.

This is a work from home position.

Car and good driving record required.
