Responsible for assessing, planning, directing, coordinating, evaluating and providing the patient care given to a specific group of patients utilizing the nursing process and established Hospital policies and procedures. Coordinates with the physician, other health care team members and family members to ensure the highest standard of care.

The candidate must be bilingual and possess strong verbal and written skills in addition to being culturally sensitive to underserved populations. Knowledge of community resources is required. Some public speaking may be required. Candidate must be CDE (Certified Diabetes Educator) or willing to work toward CDE status. Case management experience desirable. Candidate should be comfortable working in a self-directed environment and possess strong customer-friendly skills.
