Social Services Director

This individual will assist residents with adjustment

to placement, aid in discharge planning for

short-term placements, provide assessments,

consultation and support to residents and families,

and maintain documentation.The qualified

candidate will have a BSSW degree, state licensure,

and a minimum two years' experience in a

long-term care environment.

RN's/LPN's/CNA's - ALL SHIFTS
