Serve as Mental Health Nurse for the PACT multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to program consumers with severe and persistent mental illness under the clinical supervision of the Team Leader and psychiatrist.
1.Take the lead role or participate in providing medication administration and medical services.
a)Under the direction of the team psychiatrist and in collaboration with the other registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed but the team psychiatrist.
b)Evaluate and chart psychotropic medication effectiveness, complications, and side effects
c)Arrange for required lab work according to protocol
d) Organize and manage the system of getting medication to clients and integrate medication into client’s individual treatment plans.
e) Manage pharmaceuticals and medical supplies.
f)Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medial treatment.
g.Administer IM injections according to protocol
h.Properly destroy medications according to protocol
2.Provide case management for an assigned group of consumers.
3.Assume primary responsibility for developing, writing, implementing, evaluating and revising treatment goals and plans for an assigned group of consumers, within PACT policies and procedures.
4.Conduct comprehensive assessment of psychiatric history, mental status and diagnosis, physical and dental health, use of drugs and alcohol, education and employment, social development and functioning, activities of daily living and family structure and relationships.
5.Document consumer progress in a timely manner, as established by PACT policies and procedures, to maintain a permanent record of consumer activity according to established methods and procedures
6.Responsibly perform shift management duties according to established policies and procedures.
7.Provide on-call crisis intervention covering nighttime hours and serve as backup to evening and weekend staff.
8.Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
9.Educate and support consumer’s families and advocate for client’s rights.
10.Assist consumers in building supportive networks; establishing and maintaining connections and positive relationships.
11.Take lead role or participation in the development, implementation, evaluation or revision of group opportunities (i.e. socialization, chemical dependency, emotional management, illness management and recovery, nutrition, etc.)
12.Maintain a high standard of courtesy and cooperation in dealing with co-workers, consumers and visitors while demonstrating good teamwork skills including problem solving, cooperation, stating opinions and negotiating.
REQUIREMENTS
Requires prior psychiatric nursing experience & current RN license for Iowa. Preferred bachelor’s level. Must have own transportation & good driving record. Must be able to pass background check.

2.Must have a strong commitment to the rights and abilities of each person with a severe and persistent mental illness to live in normal community residence; work in market jobs; and have access to helpful, adequate, competent and continuous supports and services.
3.Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for consumer rights and personal preferences in treatment are essential.
4.Ability to follow directions and adhere to ethical standards applied by accrediting agencies.
5.Ability to speak, read and write fluent English with ability to write legible entries in case notes and complete required written assignments.
6.Ability to meet consumers in their homes or in public places with ability to drive safely, and a willingness to transport consumers in own vehicle, as needed.
PHYSICAL DEMANDS:
1.Have 50% of normal range auditory and visual acuity or have hearing and site correction to not less than 50% of normal range.
2.Able to lift to waist level 50 pounds and push or pull up to 100 pounds.
3.Ability to tolerate a high stimulus environment.
4.Able to climb up and down stairs.
