MDS Coordinator

Shift: 1st ShiftSummary: Provides professional nursing care. Demonstrates knowledge and skills necessary to provide care appropriate to the ages of the residents served, and ability to provide care according to departmental policies/ procedures. Provides care over an extended time, in various settings, spanning the illness to wellness continuum. Demonstrates knowledge and skills in the following areas: patient advocacy, preadmission process, projecting R UG Category and costing of services, Coordinating and facilitating the patients clinical course of treatment, evaluating clinical options, review and management of the patients clinical status, communication with the interdisciplinary team (therapy and billing office), managing the MDS assessment process to legitimately capture the highest resources, evaluating planned vs, actual services provide and tracking outcomes. Performs other related duties as deemed competent.
REQUIREMENTS
Qualifications: EDUCATION: Graduate of an accredited school of nursing is required. Experience: Long Term Care, supervisory, MDS experience highly preferred. License/Registration: Current NC license - Registered Nurse or Licensed Practical Nurse. Current CPR certification issued by the AHA is required. Other: Thorough knowledge of nursing standards, principles and procedures; excellent interpersonal skills relating to internal/external customers to obtain and interpret age-appropriate information required to assess the scope of treatment and patient care. Computer skills required. TYPICAL PHYSICAL DEMANDS: Requires full range of body motion, including handling/lifting patients, manual/finger dexterity and eye-hand coordination. Requires corrected vision/hearing to normal range. Requires standing/walking for extensive periods of time with bending, stooping/stretching. Frequently lift/carry patients and/or items weighing 50 lbs or more.
