Long-term Care Nurse

Take nursing history from patients/significant others and perform physical assessments on patients

Identify and document obvious patient problems, symptoms, behavioral changes and deviations

Implement nursing care plans and medical plans of care

Guide care given by other personnel on the nursing health care team as required and seek assistance as needed
