Licensed Social Worker
Presbyterian Homes of Roseville is seeking a Licensed Social Worker to join our Long Term Care Community. This is a full time benefit eligible position scheduled 30 hours per week. Responsibilities include admissions and discharge planning, MDS and other variable assessments, care planning and facilitating care conferences, and being a resource to residents, families and staff members. Previous experience in a long term care environment is desirable, must have strong communication skills.

REQUIREMENTS
Current state licensure as a Social Worker.
Previous long term care experience preferred.
A successful candidate will have strong interpersonal skills, a dedication to high quality care, and be a team player.
Demonstrated compatibility with PHS's mission and operating philosophies.
Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
