Cummins Behavioral Health Systems, Inc. has exciting opportunities available for a highly-motivated, innovative licensed home-based family preservation/reunification therapist and for highly-motivated, innovative care coordinators to assume key roles at our Johnson County (New Whiteland, IN) location.
The licensed home-based family preservation/reunification therapist must be able to provide diagnostic assessments, formulate consumer treatment plans and provide ongoing individual and group therapy to youth and families. Majority of services will be home-based/community-based.
In conducting intakes, this individual will serve as the initial contact with persons requesting services, convey the message of the hope of recovery while providing clinical intervention; provide a first source of clinical contact, screening, assessment, referral and emergency services to consumers and their families; convey the mutual expectations involved in the therapeutic partnership; participate in team staffing and function as a member of the interdisciplinary treatment team; and to provide community education services as needed.
This individual must demonstrate the knowledge and skills necessary to provide assessment appropriate to the age of the consumer served; knowledge of diagnostic patterns for addiction and mental health disorders; and ability to interpret all relevant assessment data and transform that data into information which appropriately links the consumer to the most appropriate care level.
Excellent organizational, communication & documentation skills are essential for this position.
This position requires an advanced degree and licensure or be licensed eligible as a licensed clinical social worker (LCSW), licensed mental health counselor (LMHC), licensed marriage and family therapist (LMFT).
The care coordinators will provide outreach, school-based, in-home, family reunification/preservation, and/or community-based care to assure continuity of care, accessibility and efficiency to meet the comprehensive needs of youth and adults with severe mental illness, addictions, and/or severe emotional disabilities. These individuals will work primarily with youth; however, adult population may also be expected. These may include services in school settings as well as home settings.
The care coordinators will utilize psychosocial rehabilitation principles and/or service standards of the Department of Child Services to assure positive relationships, personal growth, and goal orientation. These care coordinators will become an integral part of our high performance work team.
REQUIREMENTS
Graduation from an accredited university with a minimum of a Bachelor’s Degree in a human services field and/or health related field; Master’s Degree preferred.
