LPN/LVN Exemption: Non-Exempt Summary: The LPN/LVN provides dialysis therapy in the treatment of the end stage renal disease patient.Qualifications:1) Current license for LPN/LVN in the state of practice.2) IV certified if state required (i.e. California).3) CPR certified.4) Minimum of one year experience in a hospital or clinical setting (medical/surgical experience preferred).5) Ability to communicate with all levels, whether professional or non-professional.Essential Job Functions:
Patient Care:
1) Perform patient assessment: vital signs and all other parameters of ESRD patients, review dialysis orders, notes general physical and mental condition of patient through observations and interview, provides data to determine observations and interview, provides data to determine compliance to dietary or medication regimen, document observations appropriately.2) Provide chronic hemodialysis therapy: recommends a dialysis plan for ultrafiltration, initiate dialysis via various accesses, observe all safety measures, monitor patient vital signs applicable to ESRD patients, administer medications, fluid therapy and blood products or other appropriate treatments as needed, document activity appropriately.3) Administers appropriate drug dosages per physician instruction if approved by state agencies.
4) Collect patient data: assess and monitor status of vascular accesses, review lab work, patient assessments, and seeks guidance when levels are out of "normal" range for ESRD patients.5) Administer patient quality care in a considerate, respectful manner.Facility / Equipment Management:1) Prepare and maintain dialysis machines and systems.2) Maintain dialyzer records.3) Ready dialysis water treatment system for use according to DaVita procedure.4) Prepare dialysis delivery systems for use according to DaVita procedure.5) Inform technical staff of any medications.6) Obtain machine cultures as requested.Financial Management / Quality Assurance:1) Ensure changes for dialysis are recorded according to company procedures.2) Complete all flow sheets and charge vouchers.3) Check stock levels and order inventory and supplies.4) Participate in other quality assurance cost containment programs.5) Compliance with DAVITA policy and procedures in accordance with state and federal regulations for patient care and services (Medicare, OSHA, or other governing policies).6) Compliance with state or federal regulations of the Injury Prevention and Safety Training Program.Other Duties as Assigned:1) Able to work overtime with little or no notice.2) Ability to identify and report patient needs to Charge Nurse or Facility Administrator.3) Ability to lift minimum 35 pounds, and able to perform repetitive standing, sitting, stooping, walking, stretching, reading, and use full range of body motions.4) Understand and follow DAVITA policy and procedures as related to patient care and employment.DaVita Inc. is the largest provider of dialysis services in the United States for patients suffering from chronic kidney failure, also known as end stage renal disease, or ESRD.
