Internal Case Manager- Eastern Shore and Southern DE

We're focused on becoming the recognized leader in clinical quality and customer satisfaction in every market we serve.Home based position and requires weekly visits to the facilities throughout the Eastern Shore and Southern Delaware area.Act as resource to physicians, NPs and Treatment Team to identify alternate, cost-effective treatment options. Review Admission orders on all managed care patients, clearly communiate contract inclusions and exclusions to Treatment Team as necessay. Maintain and submit utilization data/information/reports as requested by Managed Care Operations or the Manager of Internal Care Management.Job Requirements
Five years of recent clinical nursing experience required. Prior experience in utilization review, case management or discharge planning required. Experience in rehabilitation nursing, acute care and/or the insurance field preferrred. Two years full time experience in case management which includes service to short/long term facility based clients preferred
