Inpatient Care Advocate Nurse (Weekday Schedule!)

As our new Inpatient Care Advocate, you would conduct chart review and discharge planning (by phone) in the Greensboro area. Primary responsibilities include:





Working closely with discharge planners and attending physicians to make sure that there is a strong discharge plan in place and that the patient is receiving the appropriate level of care

Reviewing charts to ensure the care plan meets Medicare standards

Documenting activities/case information in our electronic system

Communicating regularly with physicians, hospital staff and facility medical director

Job Requirements:

The chosen candidate must be an RN currently licensed in the state of North Carolina (Virginia RNs will also be considered). Clinical experience in med/surg or geriatrics is required. A background in case management, discharge planning and/or utilization review would be preferred, as would experience in a home health environment. Case management certification would be helpful. Candidates must have solid computer skills and effective customer service abilities.
