We seek a compassionate Social Worker to take on a rewarding role with our newly-created Hospice team. This full-time position is primarily field-based, and involves a reliable M-F work schedule. Take advantage of the growth and learning opportunities we offer, and apply today.



Key responsibilities include:


Providing end-of-life care support for patients and their families


Overseeing the volunteer program, including training and recruiting volunteers for Hospice


Coordinating crisis intervention


Maintaining contact with appropriate agencies


Overseeing admissions and social services

Job Requirements:

Candidates must possess current licensure as an LSW, LISW or LICSW. MSW preferred. A background in Hospice, Medicare, Medicaid and clinical experience in long-term care and home care are preferred. Proficiency with MS Word, Excel and the internet is needed.
