We are currently seeking qualified nurses to join our home health palliative care team.

Program: Home Care

Status: Full Time (32-40 hours/week)
Part time (24 hours per week)
Per Diem

Description:

Working with a patient population which is medically fragile with a limited life expectancy, coordinates the patient care plan and provides direct patient care to a caseload of homebound patients within a defined geographic area. Coordinates the care provided by the interdisciplinary team of primary care physicians, revisit nurses, social workers, physical therapists, occupational therapists, speech pathologists and home health aides. Receive the additional support of the Palliative Care Medical Director and the Palliative Care Clinical Pharmacist in determining optimum symptom management. In depth 3-6 month orientation and training program provided for nurses new to home health care and palliative care.

REQUIREMENTS
Licensed as a Registered Nurse in the State of California. Minimum of one year experience as a Registered Nurse preferably in an acute care hospital. Strong clinical assessment, critical thinking and patient/family teaching skills. Well organized, independent self-starter. California drivers license and automobile insurance. Current CPR certification.
