Program: Home Health

Status: Full-Time (32 or 40 hours/week)

Positions available in Redwood City South to San Jose. IV Case Manager and Palliative Care Case Manager also available.

RE-ENTRY RNS WELCOME

Description:
Develops and coordinates the patient care plan and provides direct patient care to a caseload of homebound patients within a defined geographic area. Coordinates the care provided by the interdisciplinary team of revisit nurses, social workers, physical therapists, occupational therapists, speech pathologists and home health aides. Provides hands on care which may include assessment, patient/caregiver teaching, wound care, foley cath insertion, phelebotomy, IV care.
A 3-6 MONTH ORIENTATION AND TRAINING PROGRAM IS PROVIDED TO NURSES NEW TO HOME HEALTH CARE.

REQUIREMENTS
Licensed as a Registered Nurse in the State of California. Minimum of one year of professional registered nurse experience. Strong clinical assessment and teaching skills. Well-organized, independent self-starter. California drivers license, automobile and automobile insurance. Current certification in cardiopulmonary resuscitation. Basic computer skills a plus.
