Home Health Medical Social Worker / Case Manager

Home healthcare agency seeking an energetic individual to service client in their homes in the Jefferson, Bullitt, Henry, Oldham, Shelby, Spencer County area. Part-time positions available Candidates must have home healthcare experience or willing to train.

Establishing, monitoring and delivering care as it relates to meeting the psychosocial needs of the patient/client, as directed by the physician’s plan of treatment and the interdisciplinary patient/ client plan of care. Reporting on the condition of patient / client to the appropriate supervisor and other staff members. Assisting the physician and other health care team members in assessing the significant social and emotional needs related to the health problems of the patient/client. Performing an assessment which requires substantial specialized knowledge, judgment and skill based upon principles of psychological, biological physical and social sciences. Making decisions based on individual’s education preparation and experience in social work. Assisting with coordination of care of all assigned patients/client to assess and identify needs.

Travel locally from assignment to assignment
REQUIREMENTS
Evidence of an annual TB test and other state required tests.

Education
Masters prepared graduate with a degree in social work from a school accredited by the council on social work education. State license to practice where applicable.

Ability to communicate with all levels if the work force, clinical and non- clinical
Perform and prioritize multiple functions of tasks
Read and interpret technical instructions related to the care of the patient /client
Visually and editorially observe and assess the patient
Effectively deal with multiple changes
