We have full time and per diem positions available that entail identifying appropriate community resources; coordinating medical treatment; providing supportive counseling to client and family; developing and implementing emergency plans; planning for long term care; representing client's interests and serving as an advocate; providing education to client, family, providers and care givers; conducting home safety evaluations; and assessing, implementing and monitoring home care services when needed.

REQUIREMENTS
Must have a valid Maryland RN license, two years as a professional nurse. Eldercare experience preferred. Our ideal candidate is extremely outgoing, committed to providing world-class service, and has a positive, can-do attitude. Strong and effective assessment, care management, interpersonal and customer service skills are essential.
