We invite you to apply to the following positions in one of our many Centers of Excellence:St. Clare Medical Outreach is an award winning primary care office on wheels, begun in 1997 by St. Joseph Medical Center to provide health care for the uninsured, the needy and those unable to access the health system due to language and other barriers. It offers free medical care and medications to treat health problems ranging from colds and viruses to diabetes and high blood pressure. A board-certified physician, a family nurse practitioner, a social worker, two registered nurses and a medical technician make up the staff. Members of the Spiritual Care department are also part of the team. Specialized medical care, laboratory services, radiology services and nuclear medicine are all provided free of charge by St. Joseph Medical Center and medical staff.Under the direction and guidance of the Nurse Manager, the Family Health Nurse is responsible for the following:Providing health outreach services to community members by applying the nursing process in an organized and systematic yet flexible manner.Collaborating with community members, groups, agencies or organizations to plan, organize and provide various health care services based on assessed needs.Providing for actual and anticipated health care education needs of the community.Making recommendations for referrals based on assessed needs and appropriate guidelines.Demonstrating competency in designated skill sets as determined by patient populations.Providing community based screenings.Promoting the utilization of services and offerings of the Community Outreach Department and SJMC.BILINGUAL in SPANISH and English strongly preferred.
Current RN license/eligibility in Maryland.
Current CPR, Healthcare Provider certification required.
Bachelor's Degree (BSN) Required.
3 years of Med/Surg management experience is required.
