The right candidates will be responsible for conducting individual and family therapy sessions to help meet treatment goals. Perform home visits as required to assist the family and child in foster care transition. Developing child and family focused and specialized services and working with at risk adolescent children in a community based programs. Referral to community agencies and coordination of services required. Provide individual and family counseling with cases that may be in crisis. Crisis intervention skills are a PLUS!!!
A strong knowledge of Admission and Discharge planning including Case Management will be required for the Med-Surgical Social Work cases. REQUIREMENTS
Consideration for cases requires:LGSW /LCSW in Md.Must clear all Maryland Mandated Regulations.1 year work experience in Hospital and/or Foster Care System.Working knowledge of Maryland Foster Care & Child placement system and state regulations.Demonstrate a dedication and understanding of children and adults in transition as well as support family and caretakers.
